Everyone will meet at ATC church @ TBD on Friday August 21st, 2026 from there we will travel by car to the Youth Fall Back Retreat located @ 295C Route 366 Ladysmith, Quebec
We will return to ATC around TBD Sunday August 23rd where you will pick up your teen.

Parent/Guardian Authorization
As a parent/guardian, I understand that my son(s)/daughter(s) will be under the supervision of adult youth leaders from At the Crossroads Church. I give permission for him/her to attend the Youth Fall Back Retreat located @ 295C Route 366 Ladysmith, Quebec. I understand that transportation to and from above address will be provided.
By signing this form, I acknowledge and accept the risks associated with this trip. I agree not to hold At the Crossroads Church, its staff, volunteers and/or the property owners liable for any injury, loss, or damage to my child or their property during the event.
